Dear Parents/ Guardians:
The Mineral County Health Department will begin offering seasonal flu shots at all Mineral County Schools starting Thursday, October 8th. If you are interested in having your child vaccinated, please complete the attached consent form, and return it to school with your child prior to their school’s scheduled vaccination date. The form MUST be completed in full for us to provide services to your child.
Influenza is a serious disease that can lead to hospitalizations and sometimes even death. The CDC recommends the vaccination for everyone 6 months of age and older. Depending on whether they’ve received influenza vaccines in the past, some children younger than 9 years of age will need two doses of vaccine spaced about 4 weeks apart.
Information regarding the influenza vaccine can also be found on the CDC’s influenza website including the VIS (Vaccine Information Sheet) or by scanning the QR Code shown above.
